Acute Nursing Care

What is the Service?

We offer a range of clinical nursing support including; neonatal discharges, children and young people with long-term conditions, oncology care, stoma care and those who have been prescribed intravenous antibiotics to be administered at home.

We also offer: 

Phlebotomy:
We take blood samples on behalf of GP's and consultants.

Wound Care:
We assess, treat, dress and review wounds.

Post Operative Care:
Nursing support and advice following surgery.

Training Parents and Carers:
Competency based, child specific training and support for parents and carers.

Transition Planning:
Support is provided for young people aged 14 years and over who have long term health needs, to facilitate the transition to adult health services.

Referral Process and Enquries

Referrals can be made by parents/ carers, health professionals, social care and the voluntary sector.

Referrals and enquries are processed through the Single Point of Access Team for Community Nursing and Medical Services.
