Who we are

We are a team of Children's Nurses, Adult and Learning Disability Nurses, Health and Development Practitioners and Health Care Support Workers.

What we do

We offer a family friendly, needs-led service to children, young people, their families and carers.

We provide nursing assessment, treatment and intervention for children and young people aged 0-18 in the community across the Hull and East Riding area.

We offer our service in clinics across the region, at home, school or other convenient venues as appropriate. We can also offer telephone advice, support and review.

Aims of the service

  • To deliver a high quality and safe Community Nursing Service which is personally responsive, caring and inclusive. 

  • To provide Community Nursing support to children and young people.

  • To reduce hospital visits, admissions and length of stay.

  • To promote early discharge from hospital.

  • To provide local access to Community Nursing services across the region.

Referral Process and Enquries

Referrals can be made by parents/ carers, health professionals, social care and the voluntary sector.

Referrals and enquries are processed through the Single Point of Access Team for Community Nursing and Medical Services.
