Immunisation Team

What is the Service?

The team provide a specialist immunisation service for parents, children and young people in the community. Working alongside primary and secondary nursing and medical services, supporting those families who have difficulties accessing mainstream vaccination programmes.

The service also co-ordinates and supports the delivery of vaccination programmes by School Health Teams in senior school year 8.

Aim of Service

  • Provide a range of services that address the needs of those individuals who are experiencing barriers to accessing immunisation services in Hull.
  • To contribute to the improvements in the uptake of vaccinations therefore supporting the “Be Healthy” outcomes for children and young people.
  • To provide a high quality, efficient service which addresses any concerns the child or family have about uptake of vaccination.
  • To provide information, education and training services to other CHCP Clinical Teams and GP practice staff.
  • Support Public Health Initiatives within local high risk groups

Service Referral - How to Refer and Criteria

Referrals are received from GP/Practice Nurses, Health Visitors and School Nurses. The only referral criteria are that the child or young person has outstanding immunisations, or has difficulty accessing mainstream immunisation services.

Expected Outcomes for the Service

  • To reduce the likelihood of an outbreak of infectious disease due to increased immunisation rates in children and young people.
  • To increase the uptake of the vaccination programme across the city for those who have difficulty accessing mainstream services.
  • To respond and support in the event of an infectious disease outbreak.
  • To act as an information and training resource for the Children & Young People’s teams, in relation to immunisation and vaccination programmes.

Future Developments
